The Opportunity:
Impact is a nationally available chronic disease model of care at One Medical providing a team based approach to cardiometabolic condition management using evidence-based care delivery to One Medical patients in concert with their primary care providers. Our team of RN’s work together with our clinical pharmacists, board certified health and wellness coaches and PCPs to provide advanced clinical care to our members enrolled in the Impact program.

- Reports to the RN Impact National Program Manager
What you’ll be working on:
- The RN Impact National Program nurse is clinically responsible for coverage of a caseload of Impact enrollees
- Adhere to standard work and protocols across all RN roles , meet quality measures and adhere to best practices
- Manage caseload of extended care cardiometabolic patients including patient support, data analysis, medication management in collaboration with our clinical pharmacy team and PCP collaboration re: plan of care and improved patient metric outcomes
- Provide patient education and counseling on lifestyle modifications, connected device utilization, and data interpretation for chronic condition management
- Triage and conduct chart reviews for enrolled patients , ensuring completion of preventive health screenings (e.g., colorectal cancer, retinopathy, cervical cancer). This means placing orders on behalf of the PCP and notifying the patient and PCP of care gaps that need to be completed.
- Participate in continuous learning through weekly Clinical Rounds and other professional development opportunities.
- Engage in huddles with virtual and in-office teammates to ensure seamless patient care.
- Utilization of your specific clinical training and opportunities to give exceptional care to patients virtually
Your Qualifications:
- Active RN license w/ NCLEX, active California, New York and/or compact multi-state RN licensure required, with the ability to obtain additional licenses
- A Bachelor of Science in Nursing preferred or Associate of Science in Nursing
- 2+ years of nursing experience in Primary Care or working with patients within the scope of diabetes and chronic disease management
- Certification in Diabetes Education or expectation to achieve this within the first year, CDCES certification
- Excellent clinical and communication skills
- A strong interest in using technology to deliver high quality, evidence-based primary care
- The desire to be an integral part of a rapidly growing team of clinicians dedicated to changing health care delivery
- Comfortability with the use of remote patient monitoring specifically as it relates to continuous glucose monitoring interpretation

This is a full time, virtual position.
One Medical is committed to fair and equitable compensation practices.
The base salary range for this role is $89,000 to $104,000 per year based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience, physical location (state you live in) and patient care schedule.
