Homeward is rearchitecting the delivery of health and care in partnership with communities everywhere, starting in rural America. Today, 60 million Americans living in rural communities are facing a crisis of access to care. In the U.S. healthcare system, rural Americans experience significantly poorer clinical outcomes. This trend is rapidly accelerating as rural hospitals close and physician shortages increase, exacerbating health disparities. In fact, Americans living in rural communities suffer a mortality rate 23 percent higher than those in urban communities, in part because of the lack of access to quality care.
Our vision is care that enables everyone to achieve their best health. So, we’re creating a new healthcare delivery model that is purpose-built for rural America and directly addresses the issues that have historically limited access and quality. Homeward supports Medicare-eligible beneficiaries by partnering with health plans, providers, and communities to align incentives – taking full financial accountability for clinical outcomes and the total cost of care across rural counties.
As a public benefit corporation and Certified B Corp™, Homeward’s mission and business model are aligned to address the healthcare, economic, and demographic challenges that make it challenging for rural Americans to stay healthy. Our Homeward Navigation™ platform uses advanced analytics to connect members to the right care and local resources that address social determinants of health and improve holistic health outcomes. Since many rural communities lack adequate clinical capacity, Homeward also employs care teams that supplement local practices and reach people who cannot otherwise access care.
Homeward is co-founded by a leadership team that defined and delivered Livongo’s products, and backed most recently by a $50 million series B co-led by Arch Ventures and Human Capital, with participation from General Catalyst for a total of $70 million in funding. With this leadership team and funding, Homeward is committed to bringing high-quality healthcare to rural communities in need.
The opportunity
Join us in tackling healthcare for rural America! We’re seeking a Staff Software Engineer to be a key contributor on our Homeward Navigation team, with a strong interest in building AI-native, user-facing product features. You’ll work closely with engineering, product, design, and data partners to re-architect health and care—starting with communities that have historically been underserved.
This role is ideal for a senior technical who has shipped real-world software and has experience integrating generative AI into production products (not just experimenting with it on the side).
What you’ll do
Build AI-native product features end-to-end, from UX to backend systems, using TypeScript across the stack.
Design and implement LLM-powered workflows (e.g., chat interfaces, decision support tools, automation flows) that are reliable, observable, and safe for healthcare use.
Integrate with EHRs and third parties via APIs, webhooks, and data pipelines, ensuring data consistency, correctness, and compliance.
Develop and orchestrate AI workflows using modern tooling (e.g., LangFuse, LangChain/LangGraph, n8n, Flowise or similar), with a focus on quality, flexibility, and iterative improvements.
Implement guardrails, fallbacks, and evaluation strategies to ensure AI features behave predictably and safely in production.
Solve complex debugging, testability, and deployability challenges with a pragmatic, systems-oriented mindset.
Shape the technical roadmap for AI-powered capabilities in partnership with product, data, and design teams.
Raise the bar for engineering quality through code reviews, documentation, and mentorship.
What you bring
9+ years of hands-on software engineering experience, including ownership of complex production systems.
Strong experience building applications end-to-end with TypeScript, React, and Node.js.
Demonstrated experience or strong interest in shipping user-facing AI or LLM-powered features into a product (e.g., chat experiences, RAG pipelines, intelligent workflows, decision support).
Prior experience with creating and improving LLM-assisted coding toolchains.
Familiarity with modern LLM application patterns, such as prompt orchestration, retrieval-augmented generation (RAG), model evaluation, and observability.
Experience integrating AI systems into real-world constraints: reliability, security, cost, latency, and UX.
Nice to have
A solid understanding of security and PHI, particularly in HIPAA-regulated environments..
Hands-on experience with tools such as LangFuse, LangChain, LangGraph, Flowise, n8n, Mastra, or similar (we value transferable experience over exact matches).
Experience with LLM evaluation, monitoring, and observability in production.
Familiarity with vector databases, embeddings, or multi-step AI workflows.
Prior exposure to deploying AI systems in environments with human-in-the-loop workflows
