In this role, you will play a crucial role in supporting patients through their medical journeys, ensuring they receive the necessary care and resources. You will manage a diverse caseload, collaborate with healthcare providers, and lead training initiatives for team members. Accountabilities
Responsible for assigned caseload and meeting inbound/outbound call targets
Manage reimbursement and appointment coordination for patients
Provide clinical support and guidance on the enrolment process
Review feasibility of requests for medication administration
Ensure accurate and up to date documentation for patient appointments
Lead disease-state training of new/existing team members
Provide feedback on protocols and inter-departmental relationships
Report all Adverse Events to the assigned units/departments
Requirements
Registered Nurse (RN) or Registered Practical Nurse (RPN) diploma with five (5) years of Nursing experience
Healthcare and case management experience preferred
Bilingualism (French and English) strongly preferred
Experience in oncology is an asset
Strong patient focus and critical thinking skills
Effective communication skills, both orally and in writing
Ability to multi-task and work under pressure
Proficiency in Microsoft Office programs
