Role Overview

We are hiring a Registered Nurse (RN) to support telehealth patient follow-ups, remote monitoring, and clinical coordination.

This is a structured, high-accountability role. You will not simply “check in” with patients — you will assess, document, escalate, and coordinate care with precision.

Key Responsibilities

Patient Follow-Up

  • Conduct scheduled outbound follow-up calls
  • Assess treatment adherence and symptom progression
  • Identify red flags and clinical risks

Clinical Documentation

  • Accurately document all patient interactions in EMR systems
  • Maintain HIPAA-compliant records
  • Ensure clear, audit-ready charting

Triage & Escalation

  • Recognize concerning symptoms
  • Escalate appropriately to supervising physicians
  • Communicate urgency clearly and concisely

Care Coordination

  • Act as liaison between patients and providers
  • Ensure seamless communication across teams

Required Qualifications

  • Active RN license (BSN preferred)
  • Experience in telehealth or patient call handling
  • Strong EMR/EHR proficiency
  • Excellent English communication (spoken & written)
  • Demonstrated clinical judgment
