Position Summary
The Registered Dietitian will serve as an expert in health and nutrition for the Healthmap Solutions Kidney Health Management program. The dietitian will promote healthy eating habits to assist in slowing the progression of chronic kidney disease and associated co-morbid health conditions through nutritional modification recommendations and education.
Location: 100% Remote for candidates that are licensed in Nebraska or Louisianna or Georgia or Tennessee or Nevada or Utah.
Responsibilities
- Promote nutrition, disease management and health awareness through individualized nutrition therapy, educational resources, and programs
- Provide expert explanations for general health and nutrition issues and advise individuals on healthy eating habits to slow progression of chronic kidney disease and aid in disease management
- Utilize the Nutritional Care Process to evaluate the health and nutritional needs of members in accordance with their preferences, personal goals, health, and financial capabilities to provide effective individualized care and optimize health outcomes
- Collaborate with Care Navigators to engage members with self-management plans through education and support on healthy eating habits to improve nutritional status while enhancing well -being
- Encourage adherence to provider treatment plan and Care Navigator plan of care
- Collaborate with outside agencies to ensure continuity of care
- Participate in team meetings, multidisciplinary team meetings, in service trainings, and 1:1 meeting with Supervisor
- Provide Specialty Team updates at regular staff meetings
- Identify and work through barriers to facilitate lifestyle changes while monitoring member’s progress and performing thorough follow-up
- Participate within the multidisciplinary team to promote care coordination to provide quality member-centered care
- Document in the electronic medical record per Healthmap Solutions policies and procedures
- Perform other related duties as assigned
Requirements
- Degree in foods and nutrition, dietetics, and other health-related courses; Master’s Degree preferred
- Certified as a Registered Dietitian with an active current license
- A minimum of 5 years of experience in a clinical/community setting
- Specific experience with chronic kidney disease management and dialysis care is required
- Willingness to obtain additional licenses
