Provider Development Manager

Across the United States, 6 in 10 adults – or 133 million Americans, live with one or more chronic conditions. Chronic disease is today’s leading cause of death and disability in the US and the leading driver of the nation’s $4.1 trillion in annual healthcare costs. Patients who live with chronic conditions require far more touch points than our primary care physicians have time to deliver and the result is countless health emergencies and costly ER visits that could be prevented. Workforce shortages and lack of technologies make it difficult to give patients the attention they need.

At Cadence, our mission is to deliver life-changing care to over one million patients living with chronic disease by the end of the decade. We mitigate the impact of chronic disease by using technology and a world-class clinical team to remotely monitor, manage, and support patients at home. Together with our growing network of health system partners, we deliver guideline-directed care to tens of thousands of patients today while producing best-in-class clinical outcomes.

The Cadence Health team seeks a Provider Development Manager to join our Provider Development team. As a Provider Development Manager, you’ll play a key support role in the successful rollout and ongoing engagement of Cadence’s Advanced Primary Care Model (APCM) program. You’ll assist with provider onboarding, coordinate logistics for program launches, maintain and organize materials, and ensure follow-up activities run smoothly. As you gain experience, you'll take on a more active role in supporting providers and internal teams, helping to reinforce and streamline the integration of Cadence programs into provider workflows.

This is a great opportunity for someone early in their healthcare or program coordination career who enjoys relationship-building, thrives on staying organized, and wants to grow in a mission-driven environment.

While this is a remote position, you will be expected to travel 50% of the time.

WHAT YOU’LL DO:

Provider Support & Program Coordination:

  • Support provider onboarding by coordinating meeting logistics, preparing materials, and managing follow-ups.
  • Schedule and support introductory sessions, staff training, and follow-up meetings with providers.
  • Assist with follow-up communications to providers post-launch to ensure questions are answered and workflows are functioning smoothly.
  • Pull provider panel data and assist with internal documentation, tracking, and record maintenance.
  • Keep provider participation, preferences, and status up to date in Salesforce
  • Support action item tracking and assist with documenting provider questions and preferences.

Workflow & Resource Management

  • Ensure provider-facing handouts, scripts, and patient materials are current and well-organized.
  • Help document and refine standard workflows for providers based on conversations and training sessions.
  • Assist with updating internal playbooks, scripts, and FAQs based on field feedback and evolving program needs.
  • Maintain organization and version control of materials used during onboarding and rollout phases.

Travel & On-Site Support

  • Participate in occasional travel to support in-person provider onboarding, go-live support, and ongoing relationship building.
  • As you progress in the role, take on a more active presence in provider site visits, helping to reinforce workflows and strengthen engagement.

WHAT YOU’LL NEED:

  • Bachelor’s degree in healthcare, business, communications, public health, or a related field preferred.
  • 2–3 years of experience in a healthcare, program coordination, customer success, or provider support role.
  • Exceptional organizational skills and the ability to juggle multiple tasks and priorities.
  • Strong communication and interpersonal skills.
  • Familiarity with tools like Google Workspace, Salesforce, Zoom, and Slack.
  • Willingness to travel for provider site visits and team meetings as the role evolves.
  • A proactive, can-do attitude with a team-oriented mindset.
