UHN Connected Care
UHN Connected Care is known for our ability to deliver sustainable change management programs. Born out of a variety of province-wide projects, we are focused on the healthcare experience as a whole. We have worked with and for stakeholders across the care continuum and garnered strong regional knowledge, including electronic referrals, electronic health records and emergency management systems.
Early years saw us introducing core foundational items to achieve transformation. While there was a heavy technology lift to our projects, our work has always been successful because of an integrated project and change management delivery approach. We continue to look to the future to see where healthcare needs to go in the Ontario Health Toronto Region, seeking solutions within and beyond digital health to enable what we want to do.
Today, we break down barriers to change and connect people who want to solve these problems, bringing policy makers, funders, thought leaders, clinicians and patients to the table to build solutions together. Working with our partners to bring life to great ideas, we use these ideas to create change through sustainable, shareable practices. Our team’s values are foundational to how we approach our work, which include:
UHN Connected Care changes the way that care is delivered and experienced. We do this through driving care integration and digital enablement. Our teams deploy models of care, clinical services, and digital infrastructure to address key gaps in the health system. Our domains of work include post-acute integrated model of care for short stay patients, natural occurring retirement communities (NORCs), outpatient care, virtual care, Connected Care Hub, digital health (eServices and Resource Matching and Referral [Rm&R]) and Altum Health.
POSITION SUMMARY:
We are seeking a skilled Project Manager to join UHN Connected Care’s Toronto Region Delivery Team (TRDT). The project manager is responsible for supporting project activities, solving problems and escalating risks for support. This role will support the advancement of digital health technologies that facilitate communication between providers and patients, while building efficiencies into day-to-day processes. Advancement of these technologies will include planning, deployment, evaluation, and sustainment activities as well as leading and supporting engagement with Ontario Health Teams (OHTs), health service providers, and reporting accountabilities to our regional and provincial partners. Ultimately, the project manager will support the use and adoption of technology solutions aimed at improving patient and clinician experience as well as patient outcomes. Strong organizational and communication skills, and technical acumen are essential to coordinate with various stakeholders and drive project success. The successful candidate will be highly effective at planning, implementing and delivering projects within an inter-disciplinary team environment.
