What Does The Nurse Practitioner Do Every Day?

  • Collaborates with the IDT in assuring appropriate evaluation and palliative treatment of terminal disease.
  • Consult with and assist in the IDT’s development of patient/family plan of care to assure quality care is provided.
  • Educate the patient/family regarding disease process, self-care techniques, end-of-life care.
  • Provide training to other clinical staff as needed.
  • Perform and document visits to patients/family based on a plan of care.
  • Interact with attending and consulting physicians as necessary regarding palliative care. 
  • Actively participate in the interdisciplinary team. 
  • Provide clinical assessment results to hospice physicians after making face-to-face encounters with patients. 
  • Provides on-call services to patients/families as assigned. 


Background and Education You Need to Be Successful:

  • Graduate of an accredited School of Nursing program
  • Current State license as a Registered Nurse and as an Advanced Practice Nurse
  • Current Driver’s License in the state in which the role is located
  • 2 years of experience as an APN, in a clinical care setting, with background with a hospice agency required. 
  • Comprehensive knowledge of pharmacology, palliative care, and end of life care. 
  • Exceptional communication and interpersonal skills
  • Proof of Hepatitis consent/declination.
Salary
$5 – $10 per biweekly
