Primary Care LCSW  Trellis Health  Brunswick and Portland, Maine    Trellis Health is seeking a Primary Care LCSW. This role involves direct patient care and working within the care team to offer strategic advice on care plans, emphasizing the LCSW's role in a supportive, multidisciplinary team focused on comprehensive patient wellness. This position will be based in Portland, ME to start and will transfer to the Brunswick office in the spring.  Requirements:
  • Master’s degree in behavioral health (LCSW, LPC, or LMFT).  
  • Current State of Maine Social Work license. 
  • Experience providing clinical behavioral health services to diverse patient populations, including those with chronic medical issues.  
  • Experience working in a healthcare practice that has integrated a clinical social worker or other behavioral healthcare provider into a primary care workflow. 
  • Proven ability to provide crisis intervention and utilize de-escalation techniques. 
  • Knowledge of the use of therapeutic relationships to foster patient involvement. 
  • Knowledge of health and social issues impacting diverse clients and their well-being. 
  • Skill in client advocacy and case management. 
  • Ability to interact effectively as a member of an interdisciplinary health care team. 
  • Adherence to National Association of Social Workers (NASW) Code of Ethics. 
  • Knowledge on how to assign and educate supporting roles to ensure the success of the integrated practice. 
  • Ability to work onsite Monday-Friday 8:30-5:00

  • Responsibilities:
  • Conduct comprehensive, culturally sensitive psychosocial assessments, develop care plans, and provide counseling and crisis intervention services to patients and families. 
  • Conduct telehealth visits to support and promote access to care. 
  • Document assessments, interventions, and treatment dates in medical records. 
  • Provide emotional support, assessment, and mental health interventions for patients. 
  • Participate in advocacy and help identify resources for patients. 
  • Provide information and psychoeducation about health/mental health issues to patients, families, and groups. 
  • Consult with clinic staff and community providers as appropriate on issues involving clinical concerns, resource recommendations, etc. 
  • Identify, report, and participate in the resolution of patient safety issues. 
  • Plan and implement special events/support groups for patients. 
  • Participate in team meetings to provide psychosocial assessment of patients.
