POSITION SUMMARY
The Outreach & Enrollment Coordinator plays a crucial role in engaging prospective members through a variety of outreach efforts: phone calls, community outreach, and events. The Outreach & Enrollment Coordinator will inform individuals about the Enhanced Care Management (ECM) program. This position focuses on facilitating the enrollment process, helping members navigate available services, and ensuring a seamless and positive experience. The ideal candidate will serve as a key point of contact for both current and potential members, providing guidance and assistance to meet member needs. Success in this role requires strong communication skills, exceptional organizational abilities, and a commitment to delivering outstanding service.
WHERE YOU’LL WORK
This position is hybrid and based out of the Garden Grove office (Monday through Thursday onsite/Friday remote). Standard business hours are Monday through Friday from 8:30 am to 5:00 pm.
WHAT YOU’LL DO
- Execute outreach strategies to identify, engage, and enroll potential members
- Verify eligibility for enrollment in the ECM program
- Complete and upload enrollment intake forms to member charts
- Conduct telephonic/in-person outreach, assessments, and referrals as needed
- Represent Titanium Healthcare at community events, health fairs, and meetings
- Organize and lead community health events, including informational sessions
- Identify, coordinate, and follow up on member referrals to ensure continuity of care
- Collaborate with Lead Care Managers (LCMs) regarding member progress and care needs
- Document member enrollment evidence in EHR record concisely and on time
- Perform other duties and projects assigned
WHO YOU ARE
- Fluent in English (written and verbal)
- Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear and concise manner
- Adequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation
- Ability to identify problems and use logic and related information to develop and implement solutions
- Excellent organizational skills and attention to detail
- Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures
- Ability to communicate clearly in-person, by phone, and electronically
- Frequent use of computers, keyboard, and handheld/mobile devices
- Competent with computers, email, virtual platforms, and Microsoft Office based programs
- Ability to type for extended periods
- Must be able to remain in a stationary position
- Ability to operate a vehicle and travel to meet with members around the community; attend meetings and events as required or requested
- Must be able to move around the office and/or travel throughout community
- Work may occur in homes, shelters, outdoor settings, hospitals, or community organizations, which may include exposure to pets, smoke, odors, clutter or unsanitary condition, and varying temperature conditions
- Ability to climb stairs or navigate uneven terrain in community and home environments
- Ability to lift, carry, push, or pull up to 20–25 pounds (e.g., laptop bag, forms, mobile equipment)
- Ability to bend, reach, and conduct in-person visits in non-traditional environments
- Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice
- Ability to maintain professionalism and safety in diverse environments
WHAT YOU’LL NEED
- Minimum high school diploma/GED required
- 1 year of outbound call center experience, community outreach, or canvassing
- Current and valid Driver’s License
- Proof of auto insurance
- Distraction-free home workspace with a secure internet connection

- Experience in healthcare or social services
- Bilingual (Spanish/Vietnamese preferred)
