Requirements and Responsibilities:


  • Plans and conducts individualized occupational therapy programs in a home care environment to help patients develop, regain, or maintain their ability to perform daily activities.

  • Works cooperatively with physicians, case managers and treating therapists.
  • Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
  • Requires a bachelor's degree in occupational therapy.
  • Requires state license to practice occupational therapy in Ohio.
  • Reports to the clinical manager.
