We are seeking an enthusiastic and dedicated Nurse Case Manager to join our innovative virtual health company. In this role, you will be crucial in coordinating patient care, managing complex health issues comprehensively, and ensuring effective communication among healthcare providers, patients, and their families. By collaborating with a multidisciplinary team, you will help design personalized care plans that integrate both conventional and holistic approaches, ultimately enhancing our patients' health outcomes.

We are looking to fill the following shift: Monday to Friday from 8:00am to 5:00pm.

For Denver candidates - be willing to commute to our Denver-based office at I25 and Bellview.

For Scottsdale candidates - be onsite at our client's office near Frank Lloyd Wright Blvd and the 101.

Responsibilities

  • Conduct holistic assessments of patients to create, implement, and manage individualized care plans
  • Communicate effectively with patients, providers, and families regarding health management strategies and resources
  • Provide education and support to empower patients in managing their health and understanding their conditions
  • Collaborate with interdisciplinary team members to evaluate progress and adjust care plans as necessary
  • Document all interactions and care plans accurately within our electronic medical record system
  • Advocate for patients to ensure they receive the necessary services and resources
  • Maintain a compassionate and patient-centered approach in all interactions

Requirements

  • Hold an active and unrestricted compact nursing license
  • Bring at least 5 years of nursing experience in a busy primary or family care, emergency room, ICU, or rural setting
  • Demonstrate strong clinical assessment and critical thinking skills
  • Display excellent communication and interpersonal skills
  • Be comfortable with adapting to emerging technology to streamline and improve Amaze's systems.
  • Ability to work independently and collaboratively within a healthcare team
  • Care coordination or case management in a clinical setting is preferred
  • Experience in chronic disease management is preferred

Pay Rate is $80,000 to $95,000 - depending on experience
