Your role, your impact
As Care Navigator, you will make a real difference by guiding people through complex systems and connecting them to the right supports. You’ll help clients feel heard, respected, and supported, especially during times of vulnerability. Your work will build trust, stability, and hope for individuals and families.

Assess client needs and identify appropriate supports and referral pathways across mental health, AOD, and suicide prevention services.

Provide structured, time-limited support to clients on active hold.

Conduct brief check-ins (phone, email, secure messaging) to maintain therapeutic connection and assess wellbeing.

Identify and escalate risk factors or deterioration in client presentation.

Maintain accurate client data and records.

Support client transitions between programs, ensuring continuity and stability of care.

Participate in supervision and team meetings to review caseloads and service flow.
Who we’re looking for
You’re a compassionate, skilled practitioner who thrives in a collaborative environment and is dedicated to delivering high-quality, person-centred care. You bring clinical confidence, a commitment to best practice, and a genuine desire to support people when they need it most.
You’ll bring:
- Experience within mental health and/or alcohol and other drug (AOD) settings.
- Well-developed assessment and therapeutic counselling skills, with confidence in single-session and brief interventions.
- Strong collaboration skills and the ability to work effectively with a wide range of stakeholders.
- Excellent interpersonal, verbal, and written communication abilities.
- Sound problem-solving and negotiation skills.
- The capacity to work independently while contributing positively to a team.
- Proficiency with information management and electronic client record systems.
- Strong organisational skills, with the ability to prioritise and meet deadlines.
- Knowledge of relevant systems, processes, legislation, and evidence-based interventions.
- Ability to assess and support clients experiencing moderate to severe mental health difficulties.
- A commitment to culturally safe, trauma-informed care.
- Minimum Bachelor level qualification in a related discipline
