Medical Assistant – In-Home Quality Screenings

As a Medical Assistant, you will play a key role in our Clinical Care Team by engaging directly with patients to support risk adjustment, HEDIS, and Stars program initiatives. You will conduct quality gap closure screenings in patients’ homes, ensuring accurate data collection, patient education, and a positive care experience.

This role blends clinical and administrative responsibilities, assisting providers in delivering high-quality care while supporting our mission to improve patient health outcomes and satisfaction.

Key Responsibilities

  • Perform in-home quality screenings including:
    • Bone Density
    • Diabetic Retinal Eye Exams
    • FIT Kit administration
    • HbA1c testing
    • Spirometry
    • Kidney Evaluation for Diabetes (KED) screenings
  • Support providers during in-home assessments, including:
    • Recording vital signs and managing member records
    • Reviewing medication usage and adherence
    • Documenting health maintenance and management needs
  • Conduct telephonic scheduling, confirming, and coordinating member appointments
  • Provide compassionate, professional patient engagement and education throughout the assessment process
  • Ensure accurate documentation and data capture to support risk adjustment, HEDIS, and Stars quality measures

Requirements

Required Skills and Qualifications

  • Education: High school diploma or equivalent required; completion of an accredited Medical Assistant program or certification strongly preferred
  • Experience: Previous experience as a Medical Assistant, preferably in primary care, home health, or quality screening programs
  • Clinical Skills: Proficiency in administering common screenings, recording vitals, and assisting providers with care delivery
  • Technical Skills: Ability to use medical software, electronic health records, and clinical equipment accurately
  • Interpersonal Skills: Strong communication, organizational, and problem-solving skills; ability to build trust and maintain confidentiality while delivering excellent patient care

Preferred Qualifications

  • Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential
  • Phlebotomy experience or certification
  • Bilingual skills (Spanish/English or other languages commonly spoken in member communities)
  • Experience working with Medicare and/or Medicaid populations
  • Prior exposure to risk adjustment, HEDIS, or Stars quality programs
  • Strong patient engagement skills, particularly with elderly or underserved populations

Work Schedule & Environment

  • Schedule: Monday–Friday, 8:00 AM–5:00 PM (40 hours/week; hours may vary based on patient appointments)
  • Setting: Primarily in-home patient visits with some remote administrative responsibilities
