We are seeking an MDS Coordinator to join our community committed to delivering outstanding food to our residents. As an MDS Coordinator, you will play an important role in all phases of patient care and assessment that meets or exceeds Buckner quality, service, and nursing standards. What you'll do:

  • Input data onto the RAI instrument either via computer or long-hand in a manner that meets federal and state requirements.  Address the RAI coordinator in Austin via e-mail with questions relating to the RAI process when the specific answer is not found in the RAI manual.
  • Review unit charts daily to ensure the MDS documentation is being entered accurately by the Charge Nurse as required; Check with Health Information Management daily for critical updates.
  • Conduct in-service training for licensed staff as needed regarding the MDS procedure.  Prode the ADONs a weekly charting "window" for their forthcoming MDSs as noted by the weekly MDS schedule generated and distributed by HIM.
  • Notify the ADONs immediately of the issues noted upon review of the chart while doing the MDS.  Make daily contact with the ADONs immediately of the issues noted upon review of the chart while doing the MDS.  Make daily contact with the ADONs to discuss any issues which might impact the MDS process.  
  • Notify the Director of Nursing of any issues germane to the functioning of the nursing units which may negatively impact patient care.  Coordinate with the Administrator, Director of Nursing, and QAAC chair-person regarding issues noted on the Facility Quality Indicator Profile.
  • Obtain weekly therapy updates from SNF Coordinator and Therapy Department for required MDS documentation.  Check for distribution a minimum of twice daily.
  • Atten Quality Assessment and Assurance Committee meetings and other meetings as requested.  Attend weekly Department Head and Utilization Review Meetings.

 

 What you’ll bring:

  • High School diploma or GED; Graduate of an accredited school of nursing with an active, unencumbered license as an LVN or RN.
  • Minimum one-year related experience; Requires working knowledge of general nursing theory and practice, including basic knowledge related to nursing, such as drugs, biological, physical, social and medical sciences, and their application.
  • Requires familiarity with medical diagnoses, procedures, and accepted medical treatment patterns.  Requires knowledge of procedures and techniques necessary to administer medication and treatment as prescribed by a physician.  Requires knowledge of medications and possible side effects. 
  • Requires working knowledge of principles and methods involved in in-service instruction of nursing and auxiliary personnel, in demonstrating techniques and methods of patient-care services, and in instruction of patient's needs.
  • Requires ability to keep accurate, detailed records and prepare routine reports.
  • Requires ability to walk and stand for prolonged periods of time; ability to turn, stoop, kneel, crouch, reach, push, pull, stand and walk over 75% of shift.
