We’re looking for a Manager of Clinical Care Enablement to join our team! Reporting to the Director of Care Enablement, this person will play a critical role in advancing our mission to strengthen communities from within. You will provide clinical oversight for frontline workforce training, support clinician development across markets, and ensure rigorous quality assurance across care programs. This role is ideal for a Nurse Practitioner who thrives in fast-paced environments, brings a collaborative and optimistic presence, and is deeply passionate about both training and clinical excellence.
What you’ll do
As a trusted clinical leader, you’ll ensure our, and Health Coaches are well-prepared, supported, and achieving performance expectations. In this dynamic role, you will serve as a clinical subject matter expert, leading training and feedback cycles across markets while partnering closely with the Care Product team to act on changes. You’ll oversee clinical quality assurance and clinical appropriateness of training and deliver clinical training to care team members, including Nurse Practitioners, Social Workers, Health Coaches, and Health Engagement Specialists.
Key areas you'll add value
- Lead, plan, and schedule training for Diverge Health clinicians by developing and delivering onboarding training for nurse practitioners, social workers and other clinical staff
- Develop a “just in time” clinical training ensuring the most up to date clinical guidance, workflows, and approaches for nurse practitioners, social workers, and other clinical staff.
- Collaborate with Nurse Practitioners and Social workers through regular touch points to stay updated with current challenges or questions
- In partnership with Learning and Development, design engaging, practice-based learning experiences, such as roleplays, aligned to real-world workflows and tailored learning pathways for Nurse Practitioners and Social Workers, grounded in competency models
- Provide clinical oversight and delivery of interdisciplinary training covering disease education, motivational interviewing, care coordination, new clinical programs, and our model of care for diverse patient populations.
- Ensure clinical accuracy and consistency across all training content, materials, and assessments for NPs, SWs, Health Coach Managers, and Health Coaches, including review of health coach assignments and practicums.
- Lead and facilitate clinical quality assurance for Nurse Practitioners, including conducting shadows, completing evaluation forms, and providing feedback to individuals and management; partner with clinical and market leaders to design and implement quality assurance processes for Social Workers.
- In collaboration with the Coach Success Manager, oversee clinical quality assurance for front-line roles (Health Coaches and Health Engagement Specialists) through shadowing, chart reviews, and targeted feedback; design and facilitate clinical documentation reviews to ensure compliance with market-specific requirements.
- Serve as the clinical point of contact between market clinicians and the Care Enablement team, partnering across QA, Program Integrity, Training, and Care Product to ensure effective feedback loops, continuous improvement, and knowledge sharing.
- Collaborate with Talent Acquisition to identify and assess strong Clinical Care Team candidates, contributing clinical expertise to recruiting efforts.
What you'll bring
- Clinical Nurse Practitioner Licensure Required
- 3+ years of experience in chronic condition management, preferably within primary care or outpatient settings, experience in clinical coaching model a plus.
- Experience with behavioral health management in primary care settings is a plus, though not required.
- Proven training facilitation and clinical education experience.
- Demonstrated ability to deliver training that drives performance, builds confidence, and translates into clinical outcomes.
- Strong motivational interviewing skills and ability to coach others in their application.
- Experience in Metabolic, Cardiovascular, or Pulmonary condition care preferred.
- High-energy, positive leadership presence, able to engage and motivate teams during high-growth and launch periods.
- Skilled in creating executive-ready presentations and training assets in Microsoft Word, PowerPoint, and Excel.
- Ability to manage multiple projects simultaneously, meet deadlines, and navigate ambiguity with confidence.
- Comfort with travel to markets as needed to support training and launch activities.
- Preferred: Use advanced Excel skills to track training performance, assess learning outcomes, and surface insights that guide continuous improvement

This is a full-time, exempt, salaried position. Commensurate on candidate experience, the expected base salary range for this role is $120,000 - $130,000. This position is also eligible for an annual performance-based bonus of up to 15%.
