Role Scope

We are looking for a dynamic, service-oriented Interdisciplinary Team (IDT) Coordinator who is passionate about supporting elderly persons in their journey. Under supervision of the Center Manager, the IDT coordinator ensures that IDT responsibilities are executed efficiently and effectively, including scheduling, meetings/discussions, follow-through. 

Core Responsibilities & Expectations for the Role 

  • Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong. 
  • Continue to raise the bar.  Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants. 
  • Exhibit and honor Habitat’s values. 
  • Coordinate activities and provide administrative aid for the IDT including scheduling appointments, developing power point presentations, meeting agendas, generating reports, and conducting data entry as needed. 
  • Serve as scribe for all care planning meetings and care coordination meetings. 
  • Track all service delivery requests, participants’ hospitalizations, emergency room, and skilled nursing facility admissions and discharges 
  • In partnership with the IDT, complete the monthly reassessment list to identify which staff members are accountable for completing assessments and send out reminders when assessments are due 
  • In collaboration with the Medical Records Coordinator and Quality Improvement team, upload all care plans and signature pages into the Electronic Health Record (EMR), and perform audits on IDT-related tasks to ensure compliance. 
  • Performs related duties as assigned. 

Required Qualifications:  

  • High School Diploma or equivalency 
  • Minimum of two (2) years of administrative support experience 
  • Experience using Microsoft Word, Excel, and PowerPoint 
  • Previous experience coordinating and facilitating care conference meetings. 
  • Proof of medical clearance for communicable diseases, including a TB test. 
  • Proof of all immunizations are up to date. 
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.. 

 Preferred Qualifications: 

  • A minimum of two years experience working with the frail or elderly. 
  • Previous experience assisting patients/participants with behavioral health and substance abuse issues.

Compensation:  

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $24 - $28 / hour and is bonus eligible. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
