Rockstar is recruiting for a family-owned hospice agency with a 5-star rating from Medicare and national recognition as a “Superior Performer” by Strategic Healthcare Programs (SHP). This organization is known for providing compassionate, first-class care and is seeking a dedicated Hospice RN Case Manager to join its award-winning team.
What You’ll Do
As a Hospice RN Case Manager, the candidate will bring comfort and dignity to patients and their families by:
- Completing initial and ongoing assessments to determine hospice needs
- Creating, updating, and implementing individualized care plans
- Delivering skilled nursing care, medications, and symptom management
- Supporting and educating patients and caregivers on comfort-focused care
- Serving as case manager for an assigned caseload
- Coordinating with physicians, families, and the interdisciplinary hospice team
- Participating in on-call rotation and team meetings
- Supervising and supporting hospice aides
What We’re Looking For
- Graduate of an accredited nursing program; current RN license in Missouri (or eligible)
- 1–2 years of recent acute care experience (home health/hospice preferred)
- CPR certification
- Valid driver’s license and insured, reliable vehicle
- Excellent communication, assessment, and problem-solving skills
- Compassionate approach to end-of-life care
