We are seeking a compassionate and dedicated Hospice Quality Nurse to join our team This position offers an opportunity to make a meaningful difference in patients’ lives during their most vulnerable moments, utilizing your extensive nursing expertise and passion for compassionate care.

Duties

  • Elicits recommendations regarding continuous improvement of quality and patient safety programs from staff and develops recommendations for improvement,
  • Encourages reporting of occurrences without fear of reprisal.
  • Investigates all occurrences.
  • Leads root cause analyses.
  • Leads the implementation and monitoring of quality, patient safety and performance improvement programs.
  • Develops and leads Performance Improvement education programs for all staff, including management development programs/ Ensure facility and system-wide learning around Performance Improvement
  • Supports all departments/units with identifying performance improvement initiatives for each calendar year.
  • Oversees data collection, chart abstraction and reporting activities as necessary.
  • Responsible for Quality measures identification and reporting.
  • Responsible for Quality Assurance and Performance Improvement (QAPI) reports and preparation for monthly Quality Outcomes Committee meetings
  • Incorporates information learned from tracer and survey activities into Performance Improvement programs
  • Utilizes system-wide and facility-wide dashboards to track Quality & Safety activities
  • Liaison to clinical departments identifying and assessing problems and monitoring actions.

Requirements

  • Valid nursing license (RN) in good standing with state licensing board.
  • Proven experience in hospice & palliative medicine or related fields such as critical care, ICU, emergency medicine, or medical-surgical units.
  • Minimum of five (5) years’ experience in Nursing and minimum of two (2) years of experience in Quality Assessment, Patient Safety or Performance Improvement, required.
  • Knowledge of and experience with data, analysis and reporting.
  • Experience with EMR/EHR systems.
  • Excellent communication skills for triage calls, family education sessions, case management coordination, and interdisciplinary collaboration.
  • Knowledge of infection control practices and aseptic techniques essential for safe patient care delivery.
  • Ability to work independently in home settings while maintaining high standards of safety and professionalism.
