Overview

We provide patients and their families a complete range of home care services,

Specialty home care programs also are available, including adult cardiac, maintenance therapy, orthopedic rehabilitation and much more. 

Role Purpose

Provides direct patient care activities to home care or home health patients according to physician orders in the home environment including assessment, evaluation, treatment planning, implementation and instruction/supervision of physical therapy practice.

Responsibilities

• Instructs and assists patient to perform various physical activities and in use of assistive and supportive devices such as crutches, canes, and prostheses.
• Instructs patient and family in treatment procedures.
• Re-evaluates patient and adapts treatment program accordingly to achieve maximum benefit.
• Confers with physician and other practitioners to obtain additional information, suggest revisions in treatment plan and to integrate physical therapy treatment with other aspects of patients?
