The Care Coordinator at Blueprint is a central operational and patient-experience role responsible for ensuring that personalized care plans designed by physicians and implemented by health coaches are executed seamlessly, efficiently, and on time.

This role sits at the intersection of clinical care, operations, and client experience. Care Coordinators are not clinical decision-makers, but they are critical to maintaining continuity, responsiveness, and operational excellence in a high-touch precision medicine model.

Requirements

As a Care Coordinator:

  • Serve as the primary non-clinical point of contact for assigned clients
  • Coordinate and schedule physician visits, health coaching sessions, labs, imaging, and follow-up appointments
  • Ensure timely completion of diagnostics, data collection, and intake materials
  • Track care plan milestones and follow-through across physicians, health coaches, and clients
  • Facilitate communication between clients and the clinical team, ensuring clarity and responsiveness
  • Proactively identify and resolve logistical barriers to care delivery
  • Support onboarding of new clients and transitions between care phases

Clinical Operations & Integration:

  • Operate within Blueprint clinical workflows, protocols, and escalation pathways
  • Route clinical questions appropriately to physicians or health coaches, maintaining scope boundaries
  • Monitor client engagement, adherence, and utilization patterns and flag concerns for the clinical team
  • Support case reviews, care team meetings, and internal coordination efforts
  • Maintain accurate, timely documentation within the EMR and operational systems

Quality, Metrics & Process Improvement:

  • Maintain operational quality and performance metrics, including:
    • Responsiveness and turnaround times
    • Appointment completion rates
    • Client satisfaction and experience metrics (e.g., NPS where applicable)
  • Identify inefficiencies in scheduling, communication, or workflows and propose improvements
  • Assist in developing, refining, and enforcing standardized care coordination processes
  • Support quality assurance and continuous improvement initiatives

Professional Development & Leadership (Role-Dependent):

  • Participate in onboarding and mentoring of new Care Coordinators
  • Contribute to internal training, documentation, and best-practice development
  • Foster a culture of accountability, professionalism, and client-centered service
  • Maintain composure and effectiveness in a fast-paced, high-expectation environment

Required Qualifications:

  • Bachelor’s degree or equivalent professional experience
  • Minimum 2–4 years of experience in care coordination, patient navigation, clinical operations, or healthcare administration
  • Strong organizational and time-management skills with meticulous attention to detail
  • Excellent written and verbal communication skills
  • Comfort working with EMRs, scheduling systems, and digital health platforms
  • Ability to manage multiple clients, priorities, and workflows simultaneously
  • Ability to work independently and collaboratively in a remote-first environment

Preferred Skills:

  • Experience in concierge medicine, telemedicine, or integrative healthcare settings
  • Familiarity with laboratory, imaging, and referral workflows
  • Experience working with high-expectation clients or executive-level populations
