The My Care Partners (MCP) Program delivers chronic disease management and care coordination services across South Western Sydney, supporting individuals with complex and chronic conditions to improve health outcomes, prevent avoidable hospitalisations, and remain well at home.

Our team plays a key role in strengthening multidisciplinary care, improving shared care models with general practice to deliver high‑quality, person‑centred care.

Your role, your impact

As the Clinical Lead, you will play an important role in shaping how we deliver chronic disease management across the MCP Program. You'll provide clinical leadership, guidance, and support to Registered Nurse Care Coordinators, helping build capability and ensure consistent, best‑practice care.

Working collaboratively with internal colleagues, general practice partners, and the broader care team, your responsibilities will include:

  • Conduct comprehensive health assessments, recommending treatment, education, and self‑management strategies for patients with chronic and complex conditions.
  • Lead chronic care interventions and coordinate proactive care for at‑risk patients within the MCP Program.
  • Provide clinical oversight, coaching, supervision, and training to Registered Nurse Care Coordinators.
  • Ensure compliance with clinical governance standards, the MCP Model of Care Framework, operational procedures, and quality improvement strategies.
  • Enhance communication and team-based care using digital health tools, structured case conferencing, and clinical huddles.
  • Reduce preventable hospital admissions and emergency presentations through coordinated, evidence‑based chronic disease management.
  • Support patients to set and achieve individualised health goals through effective care planning and navigation.
  • Provide culturally safe and inclusive care for LGBTIQA+, CALD, and Aboriginal and Torres Strait Islander communities.

Who we’re looking for

You’re a committed clinician with a passion for strengthening care quality, supporting others, and improving outcomes for people living with chronic conditions. You work collaboratively, value relationship‑building, and are ready to take on a leadership-focused role.

To be successful in this position, you will bring:

  • Bachelor of Applied Science (Nursing) or equivalent, with current AHPRA registration.
  • At least 4 years’ experience in chronic disease management, plus experience in clinical leadership or staff development.
  • Advanced clinical assessment and care planning skills.
  • Confidence coaching and supporting clinical staff, with strong communication and team-building ability.
  • Demonstrated ability to build effective relationships with GPs, primary health providers, and internal/external stakeholders.
  • Experience delivering inclusive, culturally responsive care.
  • Current driver's licence and the ability to travel across South Western Sydney.
