Clarvida is seeking an innovative and purpose driven individual to join our team as a Case Manager for our program.  This position will assist the clients out of our office in Mojave.

**Salary $22-$24/hr plus full benefits** 

The Case Manager acts as the liaison between the client and the employers/representatives from educational/vocational services. They provide outreach, information, referral and a full array of mental health/rehabilitation services for clients. These services include skills training, intakes, case management, coordination and monitoring of the assessment, treatment planning and follow-up. 

Essential Duties/Responsibilities 
Administrative Duties 
     
• Complete intake assessments
* Conduct outreach calls
* Enroll clients into the program
• Refer clients to applicable resources.
• Ensure one-on-one education and employment support is provided.
• Maintain client records and provide follow up contact for successful placement assurance.
• Provide job readiness training to clients.
• Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients.
• Responsible for service coordination by assisting clients in implementing their plan of care by facilitating access to and providing community resource linkage as needed
• Coordination of care with other providers such as psychiatrist, LVN, family and PCP
• Maintain at least monthly contact with clients and as needed per client needs
• Provides professional support, interventions and linkages to clients to assist in movement to a lower level of care
• Maintains daily record of work activities completing daily service progress notes and billing forms per Company and government standards and in compliance with Short Doyle/Medi-Cal requirements (MediCal and Medicare standards)
• Complete and turn in progress notes within 24 hours of service
• Continually evaluate client progress and strengths. Utilize strengths to achieve goals set
• Develop and maintain professional working relationships with agencies and service providers
• Participates in management, staff and client meetings and training activities as required
• Attend treatment team as relevant to case managers clients
• Utilize the recovery model philosophy and provide services within this framework
• Maintain all assigned charts to meet the standards of the Company, County and State and Federal regulatory agencies.
• Complete chart reviews and corrections on a timely manner
• Provide services in the community including clients homes as needed
• Other duties as assigned or necessary to support the program and/or the company.

Knowledge, Skills and Experience
  • BA degree in human services, psychology or related field and one year related experience or two years’ equivalent experience
  • Proof of: valid California driver’s license and auto insurance, as well as proof of education are required record and current CPR certification
  • Must have minimum one year equivalent prior case management or psychiatric treatment experience
  • May be required to have knowledge of rehabilitation model
  • Excellent prioritization and organization skills
  • Strong interpersonal skills and good written and verbal communication skills
  • Intermediate level PC skills required
  • Related experience: coaching, scouts, teacher’s aide, or CPS aid
  • Closely monitored with six months of training
  •  Ability to effectively prioritize and manage time.
