Care Management Technician

  1. Creates discharge packets for patients going to skilled nursing facilities. Creating the discharge packet requires working with several disciplines to gather needed documentation such as the discharge summary, MOLST, medication administration record, etc.
  2. Delivers durable medical equipment (DME) to the patient’s room for discharge and completing any associated insurance or charity paperwork.
  3. Confirms home care start of care.
  4. Delivers clothes suitable to the weather to the patient’s room for discharge.
  5. Completes patient screenings
  • High School diploma or GED is required
  • CNA certification preferred
