Bilingual Behavioral Health Patient Care Coordinator

A Behavioral Health Patient Care Coordinator plays a key role in guiding patients through the mental health care journey. This demanding position necessitates individuals who are proactive in identifying and addressing potential concerns, equipped with a solid foundation in mental health background and treatments to better understand and support patients' needs. Active listening skills are paramount for building trust and rapport with patients, allowing for effective communication and a deeper understanding of their experiences. Additionally, maintaining HIPAA proficiency is crucial for safeguarding patient privacy and confidentiality.

Requirements

What You’ll Do:

  • Conduct telephonic or video-based intake, gather relevant patient information, and identify immediate needs or concerns.
  • Coordinate and manage patient appointments, following established guidelines.
  • Maintain accurate records in EMR/EHR
  • Direct patients to appropriate remote services or support groups
  • Address concerns with empathy and professionalism, contributing to a positive patient experience
  • Knowledge of medical terminology

What We’re Looking For:

  • A minimum of 6 mths of experience as a bilingual Medical VA.
  • Proficient in Spanish and English.
  • Proficiency in medical software and EMR/EHR systems, cClinicalWorks preferred.
  • Experience in behavioral healthcare.
  • Excellent attention to detail and accuracy
  • Excellent communications skills
  • Knowledge of medical terminology.

Preferred Qualifications:

  • Psychology Studies.
