About the Role
The Advanced Practice Registered Nurse (APRN) works as a member of the Assertive Community Treatment (ACT) team, providing community-based psychiatric and medical care to individuals with severe and persistent mental illness. As the APRN, you will oversee clients’ medical needs including psychiatric evaluations, medication management, and overall healthcare coordination. You will consult with clients and their family supports regarding medical and health concerns related to mental health and substance use conditions, provide medication education, and monitor treatment progress. For optimal client success, services are delivered primarily in the local community. This role offers primarily weekday, 1st shift hours with 8-hour shifts and stability and growth working for a national agency.

Responsibilities
Conduct psychiatric evaluations and ongoing assessments for clients with severe and persistent mental illness
Provide medication management, including prescribing, monitoring effectiveness, and adjusting treatment plans
Oversee and coordinate overall medical and psychiatric care for ACT clients
Collaborate with multidisciplinary ACT team members including housing, vocational, therapy, and substance use specialists
Consult with clients and family supports regarding mental health and substance-related medical concerns
Provide education to clients and families regarding medications, side effects, and treatment expectations
Deliver services primarily within the community to promote engagement and continuity of care
Support client recovery goals to promote independence and transition to lower levels of care when appropriate
Maintain accurate and timely clinical documentation in accordance with regulatory standards
Participate in team meetings and case consultations to ensure coordinated, person-centered care

Required Qualifications
Licensed as an Advanced Practice Registered Nurse (APRN)
Psychiatric Mental Health Nurse Practitioner (PMHNP) preferred
1 year of work experience in an inpatient or outpatient psychiatric setting preferred
Valid Georgia Driver’s License and clean Motor Vehicle Record (MVR)
Clean Criminal Background Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation – child or adult)
