Our chosen candidate must have RN experience working in the hospice space or for a hospice program to be considered and will travel throughout our entire 60 mile service radius in this on call role.

Job responsibilities can include but are not limited to:
- Administer skilled nursing for all ages in their place of residence.
- Conduct patient admissions.
- Conduct routine patient visits.
- Coordinate care within a hospice interdisciplinary team to assure patient/family needs are met.
- Communicate with attending physicians, hospice physicians, and other physicians involved in the patient's care.
- Demonstrates accountability, flexibility, respect, and integrity.
- Submits all required paperwork and documentation within 48 hours.
Qualifications:
- Current Registered Nurse license in the state of Texas.
- One (1) years experience in a hospice setting required as a RN Case Manager or similar role.
- Must have a current Texas Driver's license and dependable transportation.
