The Reablement team are a community run, interim service that support people, many with complex needs due to illness or injury, to regain their skills and confidence so they can return home after a period of therapy and support.
You will assist and support customers, including those with complex needs and behaviours with active daily living skills such as personal care, and skills that will focus on them remaining independent in their own homes.
You need to be a team player who supports, assists and promotes the reablement of customers with identified needs.
The job will be varied and will include practical tasks such as tidying and cleaning up after incidents and to maintain the health and safety of you and those around you, this will also include completing risk assessments for customers and ensuring these are continuously monitored and reviewed.
We want you to create a safe and stimulating physical environment for customers, creating opportunities and choice for customer to be as independent as possible.
