Position Summary

  • The Transition Coordinator (TC) oversees post-secondary transition related planning, supports, and services provided at a campus.   The TC reports directly to the Assistant Principal of Specialized Services and works collaboratively with case managers and the post-secondary team to ensure that post-secondary plans are individualized and comprehensive. The TC should expect to split their time between working to support early transition planning for students in grades 7-11 (70%) and implementing appropriate activities for students in grades 12+ (30%).   This includes but is not limited to: building early awareness of multiple post-secondary pathways, including but not limited to secondary education, vocational training, integrated employment, independent living and community participation; implementing a strategic family engagement plan across grades 7-12th grades, establishing and maintaining community based partnerships/interagency collaboration and school staff professional development as needed.  

Duties and Responsibilities

  • Ensures design and implementation of Special Education Transition services as outlined by the Federal and State governments by providing a foundation for the administration of transition programs and services grounded in Best Practices.
  • Supports Case Managers/IEP Teams with developing Transition Plans that are a coordinated set of activities,  results-oriented, individualized/student-centered (e.g., focused on improving the student’s academic and functional achievement,  based on the student’s strengths, preferences, and interests), and include opportunities for post-secondary education employment and independent living.
  • Ensures that each IEP is developed and aligned to the Transition Plan for each student (i.e. Goals, SDIs, Related Services, etc.).
  • Develop and maintains school based relationships with community agencies and service providers focused on  transition practices in employment, education, independent living, (i.e. Adult Services, Office of Vocational Rehabilitation, Intellectual Disability Services, ect.)
  • Design and implement methods of assessing and evaluating effectiveness of transition programing and partnerships.
  • Oversee transition assessments, goal creation, services and activities in the drafting of the IEP writing process.
  • Review, analyze, and discuss student data on a monthly basis with the APSS to maximize student learning.
  • Ensure course enrollment for students with disabilities is aligned to transition goals.
  • Participate in IEP Team Meetings, as needed.
  • Teach transition related skills to students (resume writing/interview, etc.).
  • Promote implementing Evidence Based Practices in secondary transition by providing PD for staff.
  • Stay abreast of updated transition guidance at local, state and federal level.
  • Submit accurate data to state and federal agencies.
  • Coordinate post-school outcomes tracking and coordination.
  • Continued support for Early Alumni.
  • Oversee 18-21 transition services, if available at this school.
  • Other jobs and duties as assigned

Qualifications

  • Experience and knowledge working in Best Practice Transition Services.
  • Experience and knowledge working with Special education students with both mild, moderate and profound disabilities.
  • Ability to work well in a team and across teams and to communicate effectively with diverse stakeholders including:  students, parents, teachers, administrators, community based agencies.
  • Maintains high expectations for students and self.    
  • Strong leadership, personal initiative and problem-solving skills.
  • A relentless drive to provide the highest quality education for all students.

Education and Experience

  • Bachelor’s degree (Master’s preferred) and Special Education Certificate
  • Five years urban education experience is desired
  • Minimum of three years’ experience in successful Transition Planning
  • Demonstrate knowledge of postsecondary services and supports

Physical Requirements

  • Ability to physically perform the essential duties of the role – either on-site or remotely, and to work in the environmental conditions required, such as: traveling to network campuses; maneuvering in office spaces (including standing, walking, sitting for long periods of time, speaking loudly and clearly, seeing and hearing things both near and far away); stooping, kneeling, reaching file cabinets/shelves; fine finger and hand manipulation in use of computer, chalkboard, dry erase, &/or projectors; filing, faxing, scanning, coping, typing, mailing, and making phone calls; sitting for up to two (2) hours looking at a computer monitor, using a keyboard/mouse, and typing.
