Temporary Care Coordinator - Enhanced Care Management

ABOUT THIS TEMP POSITION

This is a temporary position expected to start as soon as possible. The length of the assignment will be determined, and is always dependent on business need and dates may change. While the assignment would be at the Alliance, if selected, you would be an employee of a temporary employment agency that we would connect you with.

WHAT YOU'LL BE RESPONSIBLE FOR

  • Assisting the Enhanced Health Services (EHS) team with providing health care coordination activities for Alliance members
  • Establishing and maintaining effective working relationships with provider offices, County departments and other community agencies, and internal stakeholders related to care coordination for members, disease management, and health education
  • Performing other duties as assigned

 

WHAT YOU'LL NEED TO BE SUCCESSFUL

  • Knowledge of:
    • Working knowledge of the principles and practices of managed health care and health care systems

    • The principles and practices of care coordination

    • The  diverse needs of the Medi-Cal population

    • Windows-based PC systems and Microsoft Word and Outlook and the principles and practices of customer service

       

  • Ability to:
    • Understand the workings of the Alliance, particularly the Enhanced Health Services, Utilization Management, and Member Services departments

    • Communicate the program mission, vision, and roles

    • Quickly learn and competently navigate Microsoft Excel and Alliance computer

    • Interpret, explain, and apply policies, procedures, and guidelines

  • Education and Experience:
    • High school diploma or equivalent and three years of experience related to care coordination in a health care setting which involved interacting with members, patients, and providers in meeting their service needs (an Associate's degree in Health, Social Services, or a related field or current certification as a Medical Assistant may substitute for one year of the required experience); or a combination of education and relevant work experience may be qualifying

 

OTHER INFORMATION

  • We are in a hybrid work environment, and we anticipate that the interview process will take place remotely via Microsoft Teams.
  • While some staff may work full telecommuting schedules, attendance at quarterly company-wide events or department meetings will be expected.
  • In-office or in-community presence may be required for some positions and is dependent on business need. Details about this can be reviewed during the interview process.

The full compensation range for this position is listed by location below. 

The actual compensation for this role will be determined by our compensation philosophy, analysis of the selected candidate's qualifications (direct or transferrable experience related to the position, education or training), as well as other factors (internal equity, market factors, and geographic location).

Santa Cruz County Pay Range
$32.20$33.30 USD
Merced County Pay Range
$29.25$30.30 USD
