We have a fantastic, new opportunity to join us as a Support Worker within the Lucy Adams Team! Your role is to deliver Housing Related Support to residents within Lucy Adams services by ensuring completion of person-centred risk assessments and support plans. You will be required to refer to and work alongside partner support agencies to effectively cater for all support needs presented by residents, with the aim to support and enable residents to maximise their potential and move on successfully to independent living accommodation.
We own and manage c.7,500 homes in the east of England, working closely with other agencies and stakeholders within our communities. We build on the good foundations we have in place in being a customer-centric organisation, investing in our existing homes - including our zero-carbon journey - as well as building new homes.
We’ve also concluded a review of how we work in the future, with options around home and office flexible working, as well as a generous annual leave allowance and other benefits.
Please note this a full-time (37 hours per week) role on an initial 12 month FTC. The role may require occasional weekend work but will mainly cover hours during the working week.
Requirements
Outline of Key Responsibilities...
- Provision of a person-centred approach when delivering and reviewing Support Plans and Risk Assessments for each resident within the organisations policies and procedures
- To promote, support and ensure fair access and inclusion for each resident at every point throughout the service and to ensure each resident has the opportunity to maximise their potential and achieve positive outcomes
- To contribute to team process of new referrals and completing risk assessments with potential applicants for discussion as a team
- Encourage, accompany and assist residents to access services where appropriate and identified in their Support Plan
- Sourcing and maintaining effective networks with statutory and other external agencies to benefit residents
- Assisting residents to successfully maintain their present accommodation and access appropriate future housing options by developing life and social skills
- To undertake end of tenancy tasks including void checks and room preparation for new residents
We are looking for someone who has...
- Awareness of importance of confidentiality and data protection
- Knowledge of issues leading to homelessness
- Ability to work under pressure and keep calm in potentially stressful situations
- Computer literacy and ability to learn new IT systems
- Awareness of importance of Professional Boundaries and their maintenance
- Current Driving Licence and access to own vehicle
- Experience in relating effectively with individuals in an empathetic way
- Experience of working with individuals with mental health, drug or alcohol related issues
