Social Needs Resource Specialist - Contract (Remote)

Your north star: Research, identify, and vet community-based resources to support Pomelo patients’ social needs

In this role, you will have the following responsibilities: 

  • Build materials to provide care navigation support for patients who need help connecting to social services
  • Source and research local resources to support housing, transportation, food, education, childcare, materials and employment needs for patients from pregnancy through postpartum 
  • Call local resources to confirm accuracy, eligibility requirements, and availability
  • Update and organize internal resource guides to maintain efficient accessibility and visibility for the clinical team 

Who you are: 

  • 2+ years in healthcare administration, social work, or case management   
  • Experience working with Medicaid populations  
  • Familiarity with SDOH platforms and referrals
  • Goes the extra mile for patients
  • Results-oriented operator who moves fast
  • Exceptional verbal and written communication skills
  • Proficient with technology- especially computers and phone systems.
  • Proficient with Google Suite  
  • Meticulous attention to detail
  • Comfort with ambiguity
  • Ability to work cross-functionally with clinicians
  • Passionate about maternal and child health

Bonus points for:

  • Fluent in Spanish and English
  • Familiarity with community resources in Texas

Our compensation ranges are based on paying competitively for our company’s size and industry. In accordance with New York City, Colorado, California, and other applicable laws, Pomelo Care is required to provide a reasonable estimate of the compensation range for this role. Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity. A reasonable estimate of the current hourly rate is $20 per hour
