Senior Reentry Case Manager

The Senior Reentry Case Manager will provide reentry services, including case management, court advocacy, cognitive behavioral therapy, and linkage to stabilization services for approximately 40-44 youth and young adults annually.

Applicants should have experience serving the following priority populations: Justice involved, undocumented, disconnected from education, unemployed and underemployed, public housing residents, pregnant and parenting participants, and young adults. All applicants should be able to act with a sense of urgency, confident in their problem-solving skills, and innovative in working with multi-barrier participants, who are minors and young adults.

Requirements:

  • Willing to work alongside justice partners, including the Contra Costa Probation Department and the District Attorney’s Office.
  • Culturally competent in language and lived experience, reflecting the communities we serve
  • Collaborative and solutions-oriented
  • Resilience: able to withstand or recover from difficult conditions
  • Ability to work independently and as a member of a team
  • 2+ years of case management experience
  • 2+ years’ experience working with multi-barrier youth and young adults ages 14-24 years old  and their families 
  • Experience serving undocumented youth and families, including familiarity with current immigration law
  • Experience working with disconnected high school and transitional-aged youth
  • Familiar with the framework and practice of Youth Development
  • Familiar with the concept and practice of Restorative Justice
  • Familiar with Trauma Responsive approaches and practices
  • Up to date on local, regional, and national policies impacting youth and families
  • Able to work well under pressure and seek help when needed
  • Able to work in ambiguity; innovative, flexible, nimble, self-motivated, self-directed
  • Must have a valid California Drivers License and an insured, registered, and operational vehicle to use for work purposes (mileage reimbursed)
  • To transport participants, the vehicle needs to be insured at the level of $100,000 for injury/death to one person, $300,000 for injury/death to more than one person, and $100,000 for damage to property. 
  • Ability to assess crisis situations and initiate action independently and without direction
  • Excellent organization and leadership skills
  • Excellent oral and written communication skills
  • Ability to be on-call

Responsibilities:

  • Conduct assessments; assess youth and young adults in the areas of employment, academics, skills, risk level, and behavior; identify barriers, strengths, and individual characteristics
  • Work with youth to set educational, professional, and behavioral short—and long-term goals and individual case plans to meet identified goals.
  • Meet with participants in person weekly to monthly, depending on need, to formally monitor the progress of case plans.
  • Check-in via phone call, text, and email daily to weekly for informal monitoring.
  • Responsible for maintaining case notes on each client; accurately and regularly input data into database systems, spreadsheets, and funder databases to maintain participant information in accordance with BACR’s standards and any program-specific funder(s) requirements.
  • Make referrals to other services and programs as appropriate; attend or bring the client to such activities when necessary.
  • Build relationships and maintain regular contact with parents/guardians, foster parents, teachers/school administrators, therapists, social workers, attorneys, probation officers, and other key stakeholders.
  • Attend school meetings, court hearings, and other appointments influencing the clients; act as an advocate when appropriate.
  • Responsible for writing service plans that include wraparound barrier removal services and linkages, education, employment, financial, health and wellness, housing, etc.
  • Responsible for writing court reports and letters confirming client status
  • Support clients’ families as appropriate; assist with strategizing with families and lawyers when clients face charges; provide translation at court, etc.
  • Participate in coordinating collaborative case conferencing sessions; facilitate phone and in-person meetings between stakeholders that are each serving/supporting participants; collect status updates and additional data on participants from partners.
  • Build partnerships and collaborations leading to better outcomes for participants.
  • Submit quarterly reports that reflect all efforts and participant statuses for the month.
  • Represent BACR at internal/external networking and partnership meetings/events.
  • Conduct job as a youth development practitioner; act in accordance with BACR best practices and program core values and philosophy.

Location:

The Reentry Case Manager has a workspace at BACR’s Antioch Office; however, this mobile position requires traveling throughout Central and East Contra Costa County communities and serving in custody participants in youth detention facilities.

Above is the pay range for this position that we reasonably expect to pay. Individual compensation is based on various factors including experience/education, skillset, and geographic location.
