ROLE RESPONSIBILITIES - as RN, Nurse Home Care Case Manager, you oversee the nursing standards for the Caring for Family of Companies. You're responsible for the highest quality of case management and care for Clients – as well as providing high caliber Caregiver training and professional development  You maintain strict compliance with established protocols and the In Home Care licensing standards. This position reports to the Director of Nursing Services, and collaborates with an entire team of Nurses for ongoing support. This role covers EITHER the East or West side of Portland metro.

  • Oversee nursing services, including medication administration program
  • Ensure standardization and adherence of process
  • Maintain Client confidentiality
  • Review medication administration and medication error reports to maintain safety and compliance protocols
  • Review medication orders to confirm Client needs and contradictions, collaborating with providers for adjustments when needed
  • Conduct nursing and medication evaluation and assessment visits, addressing Client concerns
  • Train Caregivers on medication administration and evaluate competency
  • Train Caregivers on Nurse-delegated duties, ensuring competency and ability to deliver care
  • Travel to regional branches to fill staffing gaps, perform nursing assessments, and train on medication administration – as needed

Requirements

  • Demonstrate sense of warmth, welcome, and professionalism 
  • Demonstrate strong commitment to compliance  
  • Knowledge of best practices for handling PHI in compliance with HIPAA regulations 
  • Excellent verbal and written communication in English
  • Proficiency in the Microsoft Office Suite
  • Strong focus on accurate record-keeping and time management
  • Thorough understanding of the protection and use of Protected Health Information (PHI) following HIPPA guidelines.
  • Ability to take the initiative, work independently, and thrive in fast-paced team environments with a collaborative spirit.
