We’re looking for you.   

The Nurse Navigator manages high-risk patients to ensure optimal health outcomes and achieve quality measures. Patient’s needs are coordinated with the office to assess knowledge of deficits related to self-care, provide disease management coaching, identify needs that may require referral to community resources, and improve patient quality of health and wellness. Responsible for the Cerner EMR utilization for Nursing staff and Providers. Providers Patient Teaching support. Assists with Research protocols for patients.    

Responsibilities

  1. Connects patients with resources and support systems.
  2. Facilitates communication with healthcare staff and providers.
  3. Assists with appointments and paperwork such as FMLA.
  4. Tracks to ensure patients receive care at follow up appointments and from referrals.
  5. Decreases patients’ fear and anxiety.
  6. Identifies and utilizes appropriate social services.
  7. Identifies and assists with barriers to care: financial, communication, emotional, cultural, etc.
  8. Tracks interventions and outcomes.
  9. Arranges transportation when needed.
  10. Works with the community outreach nurse to educate the community.

Qualifications   

Required:   

  • Current Registered Nurse license to practice in Kansas.
  • Current Basic Life Support for Healthcare Providers (BLS).

Preferred:   

  • Bachelor of Science in Nursing (BSN).
