Responsibilities & Duties:
- Evaluate and treat patients using the most current technology and practices
- Communicate with the physician to decide the initial plan of care based on initial patient assessment & physician’s orders
- Successful management of a consistent case load
- Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
- Manage and educate Home Health Aides in implementing nursing services
- Ability to work in a collaborative environment with an interdisciplinary team
- Ability to travel San Antonio and surrounding area
Requirements
- Current and unencumbered license to practice as an LPN in the state of TX
- Minimum of one (1) year of nursing experience
- Current CPR certification
- Reliable transportation, valid drivers license with no DUI's, revocations, or suspensions in the last 5 years
