Touchstone Health of San Antonio is actively seeking a qualified and compassionate Hospice Chaplain to provide spiritual comfort and bereavement support to hospice patients and families. Duties & Responsibilites:
- Assessing potential bereavement needs of patients families
- Contributing to a resource list of community and church-sponsored grief support groups & activities
- Maintaining proper documentation of patient and family visits and bereavement services provided
- Assisting Bereavement Coordinator regarding pre-death bereavement assessments
- Provision of direct spiritual support to patients and families
- Educate fellow staff members to enhance awareness of the spiritual concerns of patients and families experiencing terminal illness and loss
- Maintaining proper documentation of patient and family visits
- Contributing to a resource group of clergy to whom specific aspects of spiritual care may be delegated
- Conducting and arranging for funeral and memorial services when necessary
- Collaborating with multi-disciplinary team to develop spiritual plan of care
Requirements
Qualifications & Requirements:
- Bereavement experience in varied multi-disciplinary settings
- Minimum two years experience in health or related field
- Valid Texas driver's license and insured, reliable transportation
- Demonstrating knowledge of grief process (normal/abnormal, anticipatory, bereavement)
- Awareness of community resources and agencies for referrals when appropriate
- Understanding of the CQI process and ensuring practices and actions reflect the CQI recommendations
- Ability to work effectively with a diverse ethnic and cultural patient population
