Position Overview

The Community Health Partner is a vital bridge between our senior patients and their clinical care team, implementing our nationally recognized clinical model – one that has been published in leading medical journals and adopted by some of the most prestigious institutions across the country. This role demands a passionate, empathetic individual who genuinely connects with older adults and is committed to their wellbeing. You’re someone who feels a calling to be on the front lines of healthcare, making a real impact on the lives of senior patients every day. Compassionate, curious, and dedicated, you’re passionate about finding the root causes of health issues, understanding your patients’ unique challenges, and removing the barriers that prevent them from achieving their best quality of life. As a CHP, you’ll be a trusted ally for seniors, guiding them through each step of their health journey and helping them reach meaningful goals through encouragement, education, and empathetic support.

Working both in-person at our Gainesville location and virtually through our telehealth platform, you'll help shape our patients' wellness journeys through personalized coaching and compassionate support. 

As part of our pioneering team, you’ll learn directly from renowned geriatric clinicians while having a front-row seat to healthcare innovation. This ground-floor opportunity positions you to make a meaningful difference in seniors' lives while gaining insights and experience that would typically take years to acquire in traditional healthcare settings.  

Core Responsibilities

Personalized Health Coaching

You’ll conduct warm, supportive, 60-minute coaching sessions designed to meet each senior patient’s unique needs, both in-person and virtually.
Using digital tools, you’ll gather both qualitative and quantitative data that inform our clinical model, helping us deliver highly personalized care.
You’ll design accessible, age-appropriate action plans centered on SMART goals, empowering each patient to make meaningful lifestyle changes.
Through compassionate support, you’ll build rapport and trust, giving seniors the confidence to take proactive steps toward better health.
You’ll monitor progress, celebrate small wins, and adapt action plans as patients make strides in their wellness journeys.
Proactively, you’ll reach out to patients with care and attention, ensuring they feel supported and engaged.

Community Engagement & Health Promotion

You’ll manage a caseload of senior patients, providing personalized support with kindness, respect, and sensitivity.
By leading workshops and group coaching sessions, you’ll create a sense of community that motivates patients to pursue wellness together.
You’ll connect with local senior resources to broaden the support network available to each patient.
Developing and refining programs, you’ll tailor your approach to meet the diverse needs of older adults.

Collaborative Care Team Integration

You’ll collaborate with physicians, nurses, and specialists to coordinate seamless, holistic care for each patient.
Leading wellness and behavior-change initiatives, you’ll empower seniors to develop healthier habits and a positive outlook.
In care planning meetings, you’ll share insights and align with the team on each patient’s needs.
You’ll be a compassionate liaison, strengthening communication between patients, families, and healthcare providers.
