The Role:

As a Family Resource Manager, you will play a key role in serving Hazel’s mission to ensure that every child is seen. heard, and cared for. We know that the world of healthcare can be daunting to navigate...and that’s where you come in.

You will serve as a steady hand to support families through their care program with Hazel and coordinate with community organizations, school partners, and health plans to establish plans for long term care. This means leaning in to identify and address barriers, whether they be cultural stigmas, financial constraints, or geographic limitations. In doing so, you will collaborate closely with Hazel’s physical and mental health providers as part of the care team.

This role is full-time and will operate on a 10-month schedule to align with the school calendar year. 

This is a remote role AND we are prioritizing candidates based in the PST timezone. You will have up to 10% travel for team events and company training as needed. 

What You’ll Do:

Care Coordination

  • Establish care plans that best meet the unique needs of individual families, using the clinical recommendations from our Hazel providers
  • Provide consultative guidance to families to understand their preferences and constraints in order to best match them with care options
  • Educate families on the basics of navigating the healthcare system, including using their insurance or accessing other payment options

Community Outreach

  • Conduct research to identify local resources for families
  • Outreach to those resources, including but not limited to: community clinics, health plans, schools, and providers to understand the landscape of resources in that geography
  • Establish collaborative working relationships across these parties to facilitate referrals 

Organization & Documentation

  • Maintain all documentation for families, ensuring relevant information is properly documented and maintained 
  • Maintain all documentation for community outreach and resources

Other Duties as Assigned

  • Support special projects across the team

  • Technical Savvy: You are comfortable learning how to use new software programs and completing digital documentation.

Minimum Experience

  • Bachelors in Social Work, Psychology, or related field
  • 2-3 years case management / care coordination experience, school or clinical setting preferred
  • Experience with children and under-served populations strongly desired
  • Spanish speaking preferred
  • Certification in mental health preferred (e.g., QMHP)
