The Role:
As a Family Resource Manager, you will play a key role in serving Hazel’s mission to ensure that every child is seen. heard, and cared for. We know that the world of healthcare can be daunting to navigate...and that’s where you come in.
You will serve as a steady hand to support families through their care program with Hazel and coordinate with community organizations, school partners, and health plans to establish plans for long term care. This means leaning in to identify and address barriers, whether they be cultural stigmas, financial constraints, or geographic limitations. In doing so, you will collaborate closely with Hazel’s physical and mental health providers as part of the care team.
This role is full-time and will operate on a 10-month schedule to align with the school calendar year.
This is a remote role AND we are prioritizing candidates based in the EST timezone. You will have up to 10% travel for team events and company training as needed.

Total compensation for this role is market competitive, with a base salary range of $45,000- $62,500, a 401k match, healthcare coverage, paid time off, and a broad range of other benefits and perks. Review our benefits at Hazel Health Benefits.
