Position Overview:
The Community Health Partner (CHP) serves as a vital bridge between our senior patients and their clinical care team, implementing our nationally recognized clinical model – one that has been published in leading medical journals and adopted by some of the most prestigious institutions across the country. We're seeking a compassionate individual who thrives on creating meaningful connections with seniors and is driven by the opportunity to make a lasting impact on their lives. Working both in-person in Gainesville and through our tele-health platform, you’ll help seniors navigate their health journeys through personalized coaching and empathetic support.

What You'll Do:
Patient-Centered Support & Guidance
● Conduct warm, supportive, 60-minute sessions in person and virtually to meet each patient’s unique needs.
● Utilize digital tools to gather data that informs care plans, ensuring each action plan is personalized, accessible, and goal-oriented.
● Empower patients with age-appropriate action plans, supporting them to make lifestyle changes and celebrating their successes along the way.
Community Engagement & Health Promotion
● Manage a caseload of senior patients with respect and sensitivity, fostering a sense of community and connection.
● Act as a trusted navigator to help seniors make the most of their insurance benefits, providing concierge-level support to access comprehensive care
● Build a network of local senior resources to expand available support for patients.
Collaborative Care Team Integration
● Work closely with physicians, nurses, and specialists to ensure a holistic, coordinated care approach for each patient.
● Act as a compassionate liaison, strengthening communication between patients, families, and healthcare providers to create seamless care experiences.
Qualifications:
Required Qualifications
● High school diploma or equivalent education required.
● A minimum of 1 year of work experience in a medical clinic or similar environment (can include volunteer work, internships, personal caregiving, or formal medical experience)
Preferred Qualifications
●Graduation from a nationally accredited Medical Assistant program
Plus
● Experience working with senior populations
● Experience using Electronic Health Records (EHRs)
● Familiarity with common business and communication platforms; naturally adaptable to new digital tools.
● Additional healthcare license(s): MA, LPN, CHW, CNA, etc.
