As a CEL, you will be a trusted leader who creates lasting relationships among our members, helping them navigate their health plans, connect with others, and thrive.
Key Responsibilities:
Program Leadership:
- Lead the Connect for Life program using our curriculum to build trust and improve health outcomes.
- Host virtual and in-person events that bring members together, ensuring they engage regularly and support one another.
- Provide leadership to peers and assist your manager in guiding the team as needed.
Event Planning & Hosting:
- Plan and host community-building events, coordinating logistics, member outreach, and venue selection.
- Collaborate with team members to ensure successful event execution and support peers with their event needs.
Member Engagement:
- Proactively engage with members through one-on-one home visits, phone calls, and community events to ensure participation and build lasting relationships.
- Meet monthly engagement goals and assist members in resolving healthcare needs.
Documentation:
- Record all member interactions, outreach, and attendance in Wider Circle’s digital system for effective tracking and reporting.
Leadership Identification:
- Identify and mentor community members as potential Wider Circle Ambassadors to support event coordination and strengthen member connections.
Prospective Member Outreach:
- Reach out to potential members through phone, in-person engagement, or events, developing personalized plans to enroll them in theConnect-for-Life program.
Liaison & Partnerships:
- Act as a bridge between members, their healthcare providers, and local organizations, helping them access additional services and close care gaps.
- Build partnerships with community-based organizations to expand resources for members.
Accountability:
- Achieve member engagement and growth targets, tracking success through attendance, feedback, and other metrics.
- Assist your manager by supporting Community Engagement Specialists (CES) in their roles, including training and shadowing as needed.
Additional Responsibilities:
- Conduct home visits to engage with hard-to-reach members and encourage their participation in events.
- Maintain reliable transportation and the ability to travel up to 50 miles to member homes.
Physical Demands:
To successfully fulfill this role, you must be able to perform the following essential functions:
- Safely and consistently drive to public places up to 30 miles away from your home.
- Frequently carry up to 30 pounds of supplies.
- Comfortably stand and speak publicly, projecting your voice in indoor and outdoor spaces.
- Sit, stand, and walk routinely during events to interact effectively with members.
- See and hear well enough to interact with members in person or by phone and respond to member questions.
- Use a computer to view, enter, and update information in different systems.
- Effectively communicate with members in person or virtually.
Requirements
- A passion for working with Medicaid/Medicare/DSNP populations and a commitment to creating strong, supportive communities.
- 3+ years of relevant experience in community outreach, teaching, facilitation, or volunteer work, with healthcare experience preferred.
- Strong verbal and written communication skills, with an ability to engage and inspire diverse community members.
- A proactive, team-oriented mindset with leadership qualities.
- Experience leading or supporting community programs with 500+ members.
- Ability to work independently in a remote setting, with exceptional organizational and prioritization skills.
- Reliable transportation and flexibility for after-hours and weekend work.
- Commitment to a drug-free workplace, with readiness for pre-employment drug testing and background checks.
