REPORTS TO:         Centralized Follow-Up Manager  & Director of Centralized Follow-Up
SALARY:                  $17.25-$23.00/Hour

POSITION SUMMARY
The Centralized Follow-Up Specialist functions as part of the clinical and administrative staff resource team that provides patient follow-up management and reproductive health services in accordance with PPGR policies, protocols, and procedures. Hours will reflect normal clinic operation hours, which may include evenings and weekends. Must be organized, with the ability to multi-task and provide timely communication with CFU colleagues.
 
PRIMARY DUTIES PERFORMED
Notify patients and document notification of abnormal laboratory and radiology results, as directed by clinician-created follow-up plans and standing orders for abnormal and normal results, within the guidelines of PPGR MS&Gs.
Perform reminders for recommended treatment/follow-up/referrals and document their completion.
Make appropriate appointments for patients for specific treatment and follow-up of abnormal lab results, and in-house referrals, and document completion.
Maintain and transmit CD-1 or Morbidity reports for all positive reportable communicable diseases to the Health Department.
Coordinate sharing of patient’s medical information with outside entities, medical providers and payor source in a manner compliant with HIPPA and MS&Gs.
Generate patient referrals, obtaining appointments, and linkage to care. 
Assist with prior authorizations and required follow-up for patient’s medical care and treatment to include compiling medical records to document condition and/or medical necessity.
Upload PP Direct medical records to patient’s charts daily.
Answer phones, monitor department voicemail daily, forward or return calls and as indicated, and document client information from calls.
Scan and label lab results as assigned into the EHR.
Manage incoming and outgoing medical records requests, lawyers’ requests & HEDIS requests.
Assist with screening, registering, educating TMAB patients as needed. 
Maintain ongoing and appropriate communication with all staff on the centralized follow-up & case management and patient services teams.
Provide regular feedback to the CFU Manager & Director of CFU regarding training and continuing education needs.
Annually, review referral follow-up lists & update as needed.
Perform other clerical and/or administrative duties as assigned.
Periodic travel to affiliate locations may be required.
  
QUALIFICATIONS
Excellent verbal and documentation skills.
Ability to work effectively and cooperatively with clinicians and health center staff.
Ability to demonstrate sensitivity and empathy to patients.
Knowledge of quality assurance and risk management standards.
Bilingual or Serbo-Croatian or Vietnamese.
High school diploma or equivalent required
