The Centralized Follow-Up and Medical Records Nurse is part of the staff team that provides patient follow-up management and reproductive health services in accordance with Planned Parenthood of Illinois’ policies, protocols, and procedures.
Essential Functions:
1. Notify patients and document notification of abnormal laboratory and radiology results, as directed by clinician-created follow-up plans and standing orders for abnormal and normal results. Provide this service for all PPIL sites in a manner that is consistent with the PPIL Manual of Standards and Guidelines (MS&Gs).
2. Perform ‘reminders’ for recommended treatment/follow-up/referrals and document their completion in a manner that is consistent with the PPIL MS&Gs.
3. Notify the Manager of Centralized Follow-Up and Medical Records, or in their absence, the Senior Director of Centralized Follow-Up and Medical Records or the Chief Medical Officer of plan inconsistencies or documentation not meeting PPIL/PPFA standards.
4. Provide AB follow-up and rotate through recovery room as needed. This includes assisting and monitoring patients in recovery room, taking vital signs, dispensing medications, giving verbal and written aftercare instructions and birth control information, and reporting signs or symptoms of post abortion complications to physician.
5. Assist physician with emergencies during clinic, screen patients with medical problems, and train staff on medical emergencies.
6. Make appropriate appointments in EPM for patients for specific treatment and follow-up of abnormal lab results and in-house referrals and document their completion in a manner that is consistent with the PPIL MS&Gs.
7. Provide regular feedback to the Program Manager regarding training and continuing education needs.
8. Demonstrate an understanding of and commitment to remain informed about PPIL protocols, policies, and procedures.

Other Responsibilities:
1. Assist physicians in procedure room during medical emergencies.
2. Handle "emergency" patient calls, to be shared with the licensed clinical staff or other delegated staff as assigned. This includes documentation and follow-up of all calls received.
3. Screen clients with medical problems prior to making abortion appointments in conjunction with Manager of Centralized Follow-Up and Medical Records and Senior Director of Centralized Follow-Up and Medical Records.
4. Address patient concerns on an as-needed basis as part of the team handling ongoing patient triage requests. Monitor Dashboard and emerging patient requests and coordinate with Manager of Centralized Follow-Up and Medical Records and Senior Director of Centralized Follow-Up and Medical Records.
5. Call-in original and refill prescriptions.
6. Review all abortion-related pathology reports. Deliver appropriate follow-up to patients. May include reviewing, signing off, and documenting receipt of reference lab pathology reports for PPIL’s Surgical Abortion sites.
7. Manage and track laboratory results
8. Serve as primary clinical staff responsible for answering abortion “problem” phone calls received through the contact center and calling patients for routine abortion follow up issues as requested. Ensure follow up is completed per PPIL protocols.
9. Staff the after-hours AB answering service as needed, which includes evening/weekend hours.
10. Take personal responsibility to remain informed about family planning methods, abortion issues and technology, services offered by PPIL, and PPIL protocols, policies and procedures, to give regular feedback to the Manager of Centralized Follow-Up and Medical Records regarding training and continuing education needs.
11. Coordinate sharing of patient’s medical information with outside entities, medical providers and payor source in a manner compliant with HIPPA and PPFA MS&Gs.
12. Generate patient referrals, obtaining appointments and linkage to care.
13. Manage prior authorizations and required follow up for patient’s medical care and treatment to include compiling medical records to document condition and/or medical necessity.
14. Other duties as assigned

Education: A current Registered Nurse licensed in the state of Illinois, or a current Licensed Practical Nurse licensed in the state of Illinois.
Experience: Previous experience in an OB/GYN related field preferred but not required. High proficiency in EPM; EPIC electronic records system preferred. Prior experience with Planned Parenthood of Illinois strongly preferred. Spanish speaking preferred.
