What you'll do
As a member of the PHS Housing team, we are committed to the mission and vision of providing supportive services that promote proactive social and independent living skills while employing effective business practices that support the success of the housing division. The Case Manager is responsible for the delivery of supportive services to participants of Aspen Terrace within the 30% AMI category, such as creating and monitoring Individual Service Plan (ISP) for each participant to include career laddering, wage progression, food/benefit security among others. The Case Manager is also responsible to serve as liaison with community resource agencies to coordinate needed services.

What you'll bring

(Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

 

  • Bachelor’s Degree. Relevant experience in the human services field may be substituted for educational requirements.
  • At least one-year experience in human services, preferably working with homeless adults, the economically disadvantaged, mentally ill or chemically dependent.
  • Current Washington State Driver’s License with a driving record acceptable to company's insurance carrier when using a company vehicle.
  • Ability to communicate and work effectively with staff from various backgrounds.
  • Able to demonstrate an intermediate level of computer proficiency in the use of Microsoft Word, Excel, Outlook, Access, and E-mail at the time of hire.
  • Current CPR/First AID Certification or within two months of hire.
  • Ability to make positive use of supervision, function effectively in an interdisciplinary team approach, participate in, and profit from in-service and on-the-job training.
  • Ability to maintain an attitude toward interactions with participants that is objective, free from personal bias; one that allows for perspective and flexibility in observing groups and individual behavior patterns.
  • Ability to demonstrate personal flexibility, and a professional demeanor, and be able to work toward broad goals, while at the same time, work with a vast number and variety of details

 

Preferably you'll bring

  • MA/S degree in social or behavioral science preferred.
  • Prior experience working with homeless population.
